Membership Change Request

THIS IS NOT A CANCELLATION REQUEST

Please fill out the following form to make a change to your membership. This form is required to make any changes to your membership.

1. All members are welcome to upgrade or downgrade their membership plans or packages without penalty.

2. Memberships can be upgraded at the start of the next billing cycle. However, if a member wishes to upgrade immediately their new membership plan will be prorated based on the number of days remaining in their current billing cycle at their new rate.

3. Changes cannot be made retroactively.

*Membership Changes can take up to 7 days to process.
